An End of Life Plan is a collection of documents that answer all the questions your loved ones may have around the time of your death. It records your thoughts, preferences and decisions meaning that those close to you know they are carrying out your wishes. An End of Life Plan is to help you feel prepared and relaxed about your death.
It can contain information about all, or some, of the following:
Legalities
Your will, Power of Attorney and any other legal documentation you have.
Advance Care Plan
A collection of documents showing your decisions about the care you want to receive prior to death and/or should you be unable to speak for yourself. These are your decisions to make and are based on your personal values, preferences and discussions with your loved ones.
After death wishes
A record of your choices for everything to do with your funeral, wake, ashes/burial, memorial, eulogy etc. This could include a pre-paid funeral plan or be your own plan for a DIY ceremony.
Household information
Practical details to do with the running of your household. What you do that someone new to it would need to know; from how the central heating works, through what time of day the dog is fed, to where you keep the washing powder, and many things in between!
Life admin
An overview of information about such things as your financial affairs, household bills and utilities, vehicles, work and your online presence.
Personal information
A record of all details about you including, but not limited to; official information such as your passport number and maiden name; contact details for friends; anything you would like kept private; any living legacies you may have created.
